It breaks it, and then white blood cells spend the following weeks transporting the debris to the lymphatic system, which is why the calendar matters as much as the machine.
A tattoo is permanent in the first place because of particle size. When a needle deposits ink below the epidermis, immune cells attempt to clear it and fail — the granules are simply too large to move — so they end up parked inside macrophages and fibroblasts in the dermis, visible through the skin above them. A laser pulse short enough and intense enough breaks those granules into pieces the same cells can finally handle.
The principle behind it is selective photothermolysis: energy delivered faster than a target can shed heat stays confined to that target instead of spreading into surrounding tissue. Ink granules are tiny, so the energy has to arrive extremely fast. Q-switched lasers, the long-standing workhorses of the field, deliver pulses in the nanosecond range of roughly 5 to 10 billionths of a second. Most people describe laser tattoo removal as sharper than getting the tattoo but much shorter, often compared to hot grease flicked against the skin or a thick rubber band snapped repeatedly in the same spot. The discomfort is real, and it is also brief: a coin-sized tattoo may take under a minute of laser time, and even a large piece is usually finished within fifteen minutes.
Clinics manage the sensation rather than eliminating it. Topical anaesthetic cream applied thirty to sixty minutes beforehand takes the edge off superficial stinging. A stream of chilled air directed at the treatment site, delivered by a cooling unit, both dulls the sensation and protects the epidermis, and it is close to standard in Korean clinics. What does a realistic fading timeline look like?#

Neither is smooth. Fading arrives in uneven steps, and the visible jump between session two and session three is usually larger than the one between session eight and session nine.
Immediately after a pass, treated ink turns chalky white for several minutes, a reaction called frosting that comes from gas released as the laser fragments the pigment. Swelling and redness follow within the hour, blistering and pinpoint bleeding are common and expected on denser areas, and the skin scabs and settles over roughly one to two weeks. Complete clearance is also not something any clinician can promise in advance, and a residual shadow is a normal outcome rather than a failure.
Hypopigmentation is a patch left lighter than the surrounding skin, and it is the more difficult of the two pigment changes to improve once established.
