GLP-1 Hair Loss: Causes and What to Do
How to recognize likely shedding, rule out other causes, and choose a safe next step
GLP-1 hair loss is a real report in weight-loss trials, but that does not prove the drug directly damages hair follicles. Rapid weight loss and lower food intake can trigger temporary diffuse shedding, while thyroid disease, iron deficiency, illness, hormonal changes, and inherited hair loss may look similar. Do not stop semaglutide, tirzepatide, or another prescription on your own. Record when shedding began, how your weight and eating changed, and whether the loss is diffuse or patchy, then review the pattern with the prescriber or a dermatologist.
Is GLP-1 hair loss caused by the medication or weight loss?#
The most accurate answer is that more than one pathway may contribute, and current evidence cannot assign every case to the medication itself. Hair loss was recorded more often with active treatment than placebo in trials, yet the timing and diffuse pattern often fit telogen effluvium, a known response to substantial weight loss or physiologic stress.
The February 2026 US Wegovy prescribing information reports hair loss in 3% of adults receiving semaglutide injection for weight management and 1% receiving placebo. The February 2026 US Zepbound prescribing information reports hair loss in 5%, 4%, and 5% of participants receiving the three studied tirzepatide maintenance doses, compared with 1% on placebo. The Zepbound label specifically says these reactions were associated with weight reduction; it does not establish a follicle-toxic mechanism.
Telogen effluvium is temporary, diffuse shedding that follows a physical or emotional stressor. A larger-than-usual group of hairs shifts from the growth phase into the resting phase, then sheds later. The American Academy of Dermatology's guidance on hair shedding lists losing 20 pounds or more among common stressors and notes that people often notice shedding a few months after the event.
A 2024 original study in Annals of Dermatology reviewed 140 patients diagnosed with weight-loss-associated telogen effluvium. Average weight loss was 15.2%, average loss rate was 3.54 kg per month, and the patients improved after an average of 4.83 months of follow-up without treatment. This single-center retrospective study describes people who already had the diagnosis; it does not define a safe weight-loss threshold or predict who will shed.

What does temporary shedding usually look like?#
Telogen effluvium usually causes increased shedding across the whole scalp rather than a single bald patch. Hair may collect in the shower, brush, or on clothing, while the scalp itself looks relatively normal. The delay between the trigger and visible shedding is an important clue, not a guarantee.
AAD guidance says normal shedding is about 50 to 100 hairs daily, though trying to count every strand is usually impractical. A more useful record is a weekly photo of the center part and temples in the same lighting, plus notes about shower-day shedding, medication changes, illness, menstrual changes, and the pace of weight loss.
The AAD's shedding timeline says excessive shedding commonly becomes noticeable a few months after a stressor. When the body readjusts, shedding often stops and normal fullness tends to return within six to nine months. Recovery can take longer if the trigger continues or if telogen effluvium overlaps with female- or male-pattern hair loss.

Should you stop a GLP-1 medication because of hair loss?#
Do not stop, skip, split, or change the dose without speaking with the prescribing clinician. Hair shedding alone does not reveal whether the prescription is the cause, and an abrupt change may disrupt blood-sugar or weight-management care. The decision should account for why the medication was prescribed, how severe the shedding is, other symptoms, weight trajectory, nutrition, and alternatives.
Use this decision aid to choose the next conversation:
- Diffuse shedding without scalp pain or bald patches: contact the prescriber for a routine review and keep taking the medication as directed until that discussion
- Rapid loss, very limited intake, vomiting, or difficulty meeting basic nutrition needs: contact the prescriber or a registered dietitian soon
- Round patches, eyebrow loss, scalp redness, scale, pain, sores, or scarring: arrange prompt evaluation by a dermatologist
- Fatigue, cold intolerance, heavy menstrual bleeding, or other new systemic symptoms: ask whether targeted testing for non-drug causes is appropriate
- Severe medication symptoms or inability to keep fluids down: use the urgent-care instructions supplied by the prescriber rather than waiting for a hair appointment
A clinician may review the medication timeline, rate of weight change, dietary history, family history, scalp, and hair shafts. Testing is not identical for everyone. Depending on the history, a clinician may consider a complete blood count, iron studies, thyroid testing, or other targeted tests rather than ordering a broad supplement panel.
Bring the product name, current prescribed dose, start date, dose-change dates, weight trend, and a list of all medications and supplements. Photos taken before shedding started are especially useful. These details make a medication-benefit conversation more informative than a bag of shed hairs or an estimated daily count.
Can protein, iron, biotin, or hair supplements help?#
Adequate nutrition matters, but supplements are not a universal fix for GLP-1 hair loss. Reduced appetite can make it harder to consume enough protein, iron, zinc, and overall energy. The safer approach is to identify an actual intake gap or deficiency and correct it with a clinician or registered dietitian, not to take high doses “just in case.”
Build meals around foods you can tolerate and eat consistently. Examples include eggs, yogurt, fish, poultry, tofu, beans, and lentils, paired with fruits, vegetables, grains, or other energy sources. There is no single protein target that fits every adult: body size, kidney function, age, activity, food tolerance, and medical conditions all affect the appropriate plan.
The AAD lists inadequate biotin, iron, protein, or zinc among possible contributors to noticeable hair loss. That does not mean everyone with shedding is deficient. A 2017 clinical review of diet, nutrient deficiency, and hair loss concluded that deficiencies should be corrected when present, but evidence for supplementation without documented deficiency is limited and some supplements can cause toxicity or even worsen hair loss.
Iron should not be started solely because hair is shedding. Excess iron can be harmful, and ferritin results need clinical interpretation because inflammation and other conditions can affect them. High-dose zinc can interfere with copper status. Biotin deficiency is uncommon in people eating a varied diet, and biotin supplements can interfere with certain laboratory tests. Tell the clinician and laboratory about every supplement before testing.
Topical minoxidil may be appropriate for some diagnosed hair-loss patterns, but it is not automatically needed for short-lived telogen effluvium. A dermatologist can discuss expected initial shedding, long-term use, pregnancy considerations, scalp irritation, and whether the pattern supports treatment.

- How long does GLP-1 hair shedding last?
- When the pattern is acute telogen effluvium and the trigger resolves, the AAD says hair often regains normal fullness within six to nine months. The 2024 retrospective weight-loss study found improvement after an average of 4.83 months of follow-up. Individual recovery varies, especially when weight loss, low intake, illness, stress, or pattern hair loss continues.
- Will hair grow back if I stay on the medication?
- It may if the shedding is telogen effluvium and the underlying stressor stabilizes, but regrowth cannot be predicted from the medication name alone. Continued worsening, a widening part, a receding hairline, or scalp symptoms can point to an overlapping condition. A prescriber and dermatologist can help decide whether continued treatment, nutrition changes, testing, or hair-specific treatment fits the situation.
- Should I take biotin for GLP-1 hair loss?
- Not automatically. Evidence that biotin helps people without a deficiency is limited, and supplements can interfere with some laboratory tests. A clinician can review diet, symptoms, and risk factors before deciding whether any testing or supplementation is warranted. More is not necessarily better for hair nutrients.
